Job Overview
We are looking for a dedicated Care Coordinator to join our nursing home team. In this role, you will be responsible for overseeing the wellbeing of our residents, ensuring they receive personalized care tailored to their individual needs. Your experience and passion will play a crucial role in enhancing our residents’ quality of life while also providing support to families and staff.
Key Responsibilities
- Coordinate and manage resident care plans, ensuring they are tailored to meet each individual’s needs.
- Lead and supervise nursing staff, fostering a culture of compassion and teamwork.
- Communicate effectively with residents, their families, and healthcare professionals to advocate for the best possible care.
- Conduct regular assessments of resident health status and make recommendations based on findings.
- Organize recreational activities, incorporating fishing and outdoor activities whenever possible, to enhance resident engagement.
- Ensure compliance with all healthcare regulations and organizational policies.
- Provide training and support to nursing staff, focusing on best practices in aged care.
Qualifications
- Bachelor’s degree in Nursing or related field.
- Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO).
- Minimum of 2-3 years of experience in long-term care or nursing home settings.
- Strong leadership and organizational skills.
- Exceptional interpersonal and communication abilities.
- Ability to work collaboratively in a multidisciplinary team.
- Understand and have a passion for the needs of elderly individuals.
- CPR and First Aid certification are an asset.
